Practice Policies and Rates

 
 

Please familiarize yourself with the office policies prior to your appointment.  These policies are meant to serve you and other patients.  This is a small micropractice that is here to give you comprehensive, personalized and excellent health and wellness care, and administrative processes are kept simple to focus on what matters, your health and wellness.  For the best care, detailed health advice should be given during appointments only (either in person or by phone).

 

1. CANCELLATIONS

Please provide 48 hours notice for rescheduling or canceling your appointment.  A fee of $100 will apply if 48 hours notice is not given.  There will be no charge in case of an emergency. 

2. PRESCRIPTION REFILLS

Given COVID-19/SARS-CoV-2 outbreak, all refills are available at any time. We recommend you have a 3 month supply of long term prescriptions on hand. Please contact Dr. Silhan if you need new prescriptions and what your preferred pharmacy is. Most insurance plans have a mail order pharmacy they prefer you use for 90 day supply. These are also typically delivered to your home, limiting exposure to pharmacies.

3. LAB RESULTS

We make every effort to review labs during appointments, as the results and implications are very important and most times extensive.   Please try to have your labs done in time for your appointment if ordered ahead of time.  And please schedule your follow-up appointments to allow you to review all your results with Dr. Silhan and recommendations based on these.

4. LAB LOCATIONS

 I will make every effort to send patients to in-network labs. Please note, though, that ultimately the patient is responsible for assuring that their insurance will pay for labs.  Because Dr. Silhan is a functional medicine specialist, there are some labs which insurance does not cover or only partially covers which she will discuss with you during your visit.

Should the patient choose a lab where Dr. Silhan does not have an account, please make sure the lab results are sent directly to her. There are home draw companies in the DFW area you may wish to use in light of COVID-19 outbreak. Some will even draw for Quest in your home. They typically have a fee of $35-60 depending on what testing is requested.

5. PHONE CONSULTATIONS 

Currently, all appointments have been moved to Telemedicine, Skype, FaceTime.

6. EMAIL COMMUNICATION

E-mail communication should mostly be reserved for administrative communication, but short clarification e-mails are also fine.

7. PAYMENT

Full payment is due at the time of service by cash, check, credit card (Visa, Mastercard, etc) or debit card.

Dr Silhan is ‘out of network’ with many insurers, is currently contracted with Medicare and United Healthcare only.  When scheduling your appointment, inform us of your insurance coverage for confirmation of ‘in-network’ vs ‘out of network’. If we are out of network for your insurance, you will be billed directly, but you can submit an itemized bill (that the office will provide) to your insurance company for reimbursement for your visit.

It is important that the patient understands that insurance benefits are a matter between them and their insurance company. If the insurance company requires prior authorizations or other forms to be completed, Dr. Silhan will submit these for you, and the forms will be completed during your visit.  She will also complete FMLA paperwork or disability paperwork, and this will be done during a scheduled appointment along with the patient, or if required outside the appointment, an administrative fee of $25.00 applies.

8. FOLLOW UP APPOINTMENTS

The recommended follow up appointment date is an estimate based on the assessment at your last appointment and the expectation that the patient is doing well. If the condition changes or does not improve, if there are new lab results, if there needs to be a change to your protocol, or if there is a new health concern including acute illness, the patient should call to move up the follow-up appointment or schedule an additional appointment prior to the  regularly scheduled appointment.

9. RATES

Currently, the office is ‘out of network’ for some insurances.  Please enquire about in or out of network when scheduling your appointment.

NEW PATIENT FEES

Initial comprehensive intake visit for new patients for all conditions: $350.00. Filling out the pre-appointment questionnaire is very helpful to make your time most productive during the visit. The pre-appointment questionnaire is specific to Dr. Silhan’s practice and will be e-mailed to you in advance. The intake visit with Dr. Silhan is typically between 60-120 minutes, and she will make every effort to answer all your questions.

An initial visit will often be followed by a short visit a few weeks later to discuss the results of any bloodwork ordered during the initial visit and to start the treatment plan. 

ESTABLISHED PATIENT FEES

Dr. Silhan’s fees are based on time.

 

1.      Short 15-20 minute visit (medication refills between visits OR "pre-authorizations": $90 

2.      30-minute visit (1 condition, minimal med refills, labs): $125 

3.      45-minute visit (1-2 conditions, 1-4 refills, labs, imaging): $175 

4.      60-minute visit (more than 2 conditions, 5 or more refills, labs, +/- imaging): $250. If a follow-up appointment requires longer than 60 minutes, fee is based on time.

 

10. HOUSE CALLS

Dr. Silhan offers house calls on an individual basis after a short phone intake has been completed.  A house call fee of $250 is applied to the fees above, or higher depending on the distance of travel.  The house call fee of $250 is not reimbursed by insurance. Please inform Dr. Silhan of any acute illness before the appointment so that certain infectious precautions can be taken.
